Mental: Slot Overview

Mental is a horror slot from Nolimit City, set in a grim old asylum with a 5-reel 3-2-3-2-3 layout and 108 ways to win at the start. Reels can expand up to 20 rows, and the whole thing leans hard into creepy visuals, flickering lights and a proper unsettling soundtrack. I found it memorable straight away, but fair enough, it is a bit much if you just want something light.

Stats-wise, it sits on 96.08% RTP, extreme volatility and a max win of 66,666x the stake. Bets go from 0.20 up to 100, though some reviews mention lower local caps in the UK. It runs on mobile too, but with so much going on, I’d rather play it on a bigger screen.

Theme, Audio + Symbols

5 low symbols are body parts, skeleton hand, lungs, heart, brain and eye, while the premiums are Patient 5 up to Patient 1. The top premium pays 5x for five of a kind, and even the low symbols can chip in with small wins from 0.1x to 1.25x depending on the symbol. Wins land left to right on adjacent reels.

Skull is the Wild, then you’ve got Scorpion and Spider as the trigger symbols for the different free spins setups, plus the Dead Patient symbol for one of the key mechanics. I like how the symbols fit the theme, but I’m not entirely sold on spending a long chill session staring at lungs and asylum patients, mate. It’s well made, just not exactly cosy.

Bonus Features

Fire Frames are what drive the base game. On every spin, 1 to 13 positions can be set on fire, and any symbol inside gets split into 2 parts. That boosts the ways to win and can wake up the Enhancer Cells under reels 2 and 4, which trigger at 4 and 6 Fire Frames.

Those Enhancer Cells can reveal a wild, a patient, a Dead Patient, xWays, xSplit or xNudge Wild. xWays shows 3 matching symbols, xSplit splits symbols and turns itself into 2 wilds, and xNudge Wild nudges into view while adding +1 multiplier per step. On top of that, a spider on reel 2 or 4 triggers Mental Transform, changing into one of those useful symbols.

Dead Patient is where things get a bit mad. Hit 2 Dead Patient symbols and one reveals a premium patient with a random value from x5 up to x9,999 attached. That value is tied to symbol count rather than acting like a normal win multiplier, so the rules can feel a bit knotty at first. The base game can drag a bit when none of these pieces line up.

The Scary Free Spins Feature

3 Scorpion symbols on reels 1, 3 and 5 trigger 8 Autopsy Free Spins. If a Scorpion lands in a Fire Frame, you get +1 extra spin. During the feature, up to 5 positions get Fire Frames and they stay sticky, which makes this the easiest bonus to understand and probably the best place to learn the slot.

Add 1 Spider to those 3 Scorpions and you get 9 Lobotomy Free Spins. Here, the spider stays sticky and triggers Mental Transform every spin, plus the Dead Multiplier system starts collecting the highest value for each patient symbol and applying it to new matching patients, with the count dropping by 1 after each transfer.

Hit 3 Scorpions and 2 Spiders in the base game and you land 10 Mental Free Spins, the top version. Both spiders are sticky, Mental Transform keeps firing, and the big difference is the Dead Multiplier count does not drop after it transfers. Outside the UK, Bonus Buy can jump you straight in for 80x, 200x, 1,000x or a 220x lucky draw. UK players won’t get that option, so fair enough, it’s base game grinding here.

💬 What Players Say
Players tend to rate it highly for originality, atmosphere and the stacked bonus system, but loads of reviews also say the rules can feel overwhelming and the grim theme is not for everyone. Based on 8 reviews.
Ryan Cole’s Verdict

Mental: Slot Verdict

Overall, I’d say Mental is proper clever, proper dark, and definitely not a casual background spinner. The atmosphere is brilliant if you like horror, and the feature set has loads going on once it clicks. When it pops off, it feels wild in a way not many slots do.

Still, I wouldn’t call it an easy relax-and-play game. The rules are dense, the volatility is savage, and the theme is grim enough that some players will bounce off it fast. For me, it’s worth a go if you fancy something different and don’t mind a learning curve. For a laid-back pub-session slot, not really. For a tense, curious, high-risk spin, yeah, not bad at all.

Mental FAQ
How do Fire Frames work in Mental?
On each base game spin, 1 to 13 positions can get a Fire Frame. Any symbol landing there splits into 2 parts, which increases the ways to win and can help activate the Enhancer Cells.
What triggers the three free spins modes?
Autopsy Free Spins need 3 Scorpions on reels 1, 3 and 5. Lobotomy Free Spins need 3 Scorpions plus 1 Spider, and Mental Free Spins need 3 Scorpions plus 2 Spiders in the base game.
What is the difference between Lobotomy Free Spins and Mental Free Spins?
Both use sticky spiders and repeated Mental Transform effects. In Lobotomy, the Dead Multiplier count drops by 1 when it transfers to a new patient symbol, while in Mental Free Spins that count does not decrease.
Is there a bonus buy in this slot, and what does it cost?
Yes, where allowed. The buy prices are 80x for Autopsy, 200x for Lobotomy, 1,000x for Mental Free Spins, plus a 220x lucky draw with 50% Autopsy, 40% Lobotomy and 10% Mental odds. UK players do not get this feature.
What is the max win in Mental, and how is it named in the game?
The top win is 66,666x your stake. The game calls that outcome "This is Mental", and it is the hard cap for the slot.
